Very uneven footpath

In 3 places all close together the footpath is a clear hazard to pedestrians. There have been 3 old repairs due to tree roots but the problem has occured again in all three places. My wife has had her hip replaced. I have measured the rise accurately using a spirit level and ruler. The raise is minimum 4cm. Even a trip hazard height of 2.5cm is deemed legally actionable by the Courts. This needs urgent attention.
