Asphalt damage creates 2 trip hazards

Sent to Peterborough City Council less than a minute later. Council ref: 5544783.
Asphalt worn away at bar & just beyond, asphalt is raised by tree root. I caught my foot on the 1st hazard & hit the 2nd before I could regain my balance. I fell headfirst onto the path & needed treatment at A&E. 10 stitches in a deep cut on left knee, badly wrenched right shoulder & right wrist, broken glasses, black eye, split lip, bruising & possible nerve damage to face, severe bruising on right wrist, plus bruising to both knees & arm. Still unable to fully flex left knee & it is scarred.
